I've been watching a lot of Jeremy Kyle on YouTube and the featured videos have then led me onto US versions of what Jezza is and I found this one of Jerry Springer:

People say that Jezza is controversial but you don't see stuff as extreme as this on his shows.

From what I know, it started out with real families and stuff, but the goal of the show is to be a whacky out of control reality show. Its not supposed to be taken seriously. Guests are aware of what they are expected to do when they go on the stage, so they are supposed to be dramatic and screaming. A lot of interviews I've seen online with guests from the show has explained it's staged to a point.

I saw a few episodes in the past though that I'm pretty sure weren't staged (mostly because there was no extreme fighting in those eps)
